Holistic health care waiver for pre school?

My daughter is entering a new pre school and since were holistic, we need a waiver stating that I have an alternative health physician as opposed to a medical doctor. After typed up, My holistic Dr. is supposed to sign it and I get it notarized. How should I word this document?

“This is to advise that we reject modern medical science and thus have opted allow our daughter’s health care to be managed by quacks, frauds and charlatans. If our daughter should become ill at school, please contact the undersigned quack who is our primary health care therapist and chakra balancer.

We confirm that our daughter has been fully vaccinated with magic homeopathic water. We also advise that our daughter may be required to bring non-prescription medicines to school from time to time, which may include energy crystals, magnetic reiki socks and various untested herbal potions.

It may be necessary for our daughter to miss some lessons while she is undergoing colonic detox treatments to rid her body of nasty accumulated school toxins.”
