how can i get better HS grades?

I am a freshman in HS, and have a horrible record of bad grades. last year and 7th grade i had streight C’s and lower. this year, 6 weeks in, already im slacking. every night i go to bed, saying im going to do better, and never seem to follow through, no matter how much i tell myself i will. neither of my parents went to college, and my dad wants me to do the best i can. i WANT and would love to become a holistic doctor, or do something along the lines of medicine. i was wondering if anyone could help me out on this one. i attend a charter school, and im addicted to surfing…which has seemed to get in the ways of my studying for the past 3 weeks. i surf friday thru sun, and stay up til 3 in the AM doing HW sun night. HELP
